New Horizon Health and Social Care is a supported accommodation service based in Feltham, UK, focused on providing essential support to young people aged 16 to 18 as they navigate the critical transition to adulthood. The focus of our services is on fostering independence and equipping young individuals with the necessary skills to thrive.
Our mission is to empower young people by offering a safe, nurturing environment where they can develop life skills, gain confidence, and prepare for independent living.
How We Support Young People
At New Horizon Health and Social Care, we adopt a person-centred approach by assigning each young person a dedicated key worker who meets with them weekly to review progress and plan for future development.
Essential Life Skills
We support young people in learning how to use electrical appliances, doing laundry, cooking & meal planning, storage of food and cleanliness.

We offer supported accommodation built around the needs of young people. Among other advantages, our homes are fully furnished and offer good 24-hour security and maintenance.

With support workers on site 24/7, New Horizon Health And Social Care supports young people in their transition into adulthood. Each young person gets support tailored to their needs, with periodic review and adjustments.
